FROM ONE VETERAN TO ANOTHER

Chiropractic Care for Veterans, from a Fellow Veteran

You volunteered your body, and your life, for this country and the people in it. Taking care of you now is something I’m genuinely honored to do.

I served in the Army National Guard, and I know what it means to show up for the people beside you and do things the right way. That’s how I approach VA Community Care: the right paperwork, straight answers about what your referral covers, and care chosen for what we actually find.

Whether it’s your back, your neck or a joint that’s carried a lot of miles, we’ll start by understanding what your body needs to do. And if something else is the better answer, I’ll tell you, even if that means care isn’t here.

You don’t have to navigate your health alone. I’m honored to be part of your team.

HOW REFERRALS WORK

How to Use Your VA Community Care Referral at Excelsior & Grand

Four steps from your VA provider to our table. The referral comes first.

01

Talk to Your VA Provider

Ask your VA provider for a chiropractic referral through VA Community Care. You can ask for Excelsior & Grand by name, and some VA providers recommend us to Veterans who live nearby.

02

Your Referral Is Approved

Once it’s approved, your VA provider sends the referral to us, usually by fax. The referral has to be requested and approved through the VA before your first visit.

03

Schedule Your Visit

Book a new patient appointment online or call us. Your first visit is your history and exam, and your findings and plan are reviewed at your follow-up visit.

04

Care, Billed to Optum

We bill Optum, the VA’s Community Care Network administrator, for your authorized visits. If you need more visits, you’ll request them through your VA provider.

BEFORE YOUR FIRST VISIT

What to Know About Your VA Referral

VA Community Care lets eligible Veterans see a community provider, like Excelsior & Grand, when their VA provider refers them. A few details make the process smoother.

Questions about your referral? Call us at (952) 920-9247 and we’ll help you understand the next step.

Good to Know

  • The referral comes from the VA. We can’t request or submit a referral on your behalf. It needs to be requested and approved through your VA provider before your appointment.
  • Your referral sets your visits. The number of visits depends on what your VA provider authorizes.
  • Need more visits? Go back to your VA provider to request them. We’ll let you know when you’re getting close.
  • We bill Optum directly. Optum is the VA Community Care Network administrator for our area.

For questions about your eligibility or what your VA Community Care benefits cover, check with your VA provider.

THE E&G STANDARD

Care That Respects Your Service

Your care here follows the same standard as every patient’s: understand first, choose the treatment after the finding, explain the reason and adjust as you progress.

For Veterans, that also means clear documentation for your VA provider and honest conversations about what your referral covers, so nothing about your care is a surprise.

You gave your body to your country. It deserves careful, thoughtful care.

  • The right paperwork. Clear records and communication with your VA provider.
  • Straight answers. What your referral covers and what happens next.
  • The right treatment. Chiropractic care chosen for what we find, not a preset protocol.
  • The right referral. If someone else is the better answer, we’ll tell you.

I will call myself a "regular" patient at Dr. Heather Flatgard's clinic, because I have been a patient since she opened her clinic in St Louis Park. She has helped me work through my condition known as "COB" aka crappy-old-back. Her depth of experience in chiropractic care has kept me moving, exercising, traveling and the functionality to complete 2 sprint triathlons. If you want to feel welcomed and stay in-condition, I recommend that you make an appointment with Dr. Heather.

Dr. Flatgard has been my chiropractor for several years now. Despite my more advanced age 😊, she respects that I'm an athlete and treats me as one. Her knowledge is vast and she's incredibly helpful and caring. I highly recommend Excelsior & Grand Chiropractic to anyone and everyone.

Dr. Heather Flatgard and Mikayla are awesome. My adjustments are effective and gentle, I feel super welcome and comfortable every time I come in. I’ve seen Dr. Flatgard for a couple of years now and she’s helped me through pregnancy and postpartum, she’s also very knowledgeable about when it’s time to make referrals for PT or imaging. Mikayla is organized, friendly, and really helpful. They’re a fantastic team and I feel better with every adjustment. Can’t recommend enough!

I’ve been coming here for a couple of years, and even after moving, it’s still worth the drive. Dr. Heather is amazing! She truly listens, never dismisses concerns, and always has a great treatment plan. I leave every visit feeling relaxed, refreshed, and energized. Micaela is wonderful too, and scheduling is super easy, especially by text. Highly recommend!

How do I get a VA Community Care referral for chiropractic care?

Talk to your VA provider and ask for a chiropractic referral through VA Community Care. Once your referral is approved, your VA provider sends it to us.

Can I ask to be referred to Excelsior & Grand?

Yes. You can ask your VA provider for Excelsior & Grand Chiropractic by name. Some VA providers also recommend us to Veterans who live near our St. Louis Park, MN office.

Can you request the referral for me?

No. We can’t request or submit a referral on your behalf. It needs to be requested and approved through your VA provider before your appointment with us.

How do I schedule once my referral is approved?

Schedule a Visit online as a new patient, or call us at (952) 920-9247. Your first visit is your history and exam, and Dr. Heather reviews your findings and plan at your follow-up visit.

How many visits does my referral cover?

It depends on the referral your VA provider sends us. Each referral authorizes a set amount of care.

What if I need more visits?

To continue care after your authorized visits, you’ll need to go back to your VA provider and request more. We’re happy to share what we’re seeing so you can have that conversation.

Is there a cost to me?

Authorized VA Community Care visits are billed to Optum, and our understanding is that there’s generally no cost to the Veteran for that care. Your VA provider can confirm the details of your benefits.

What should I bring to my first visit?

A photo ID and a list of any medications and past imaging or records you think are helpful. We usually receive your referral directly from your VA provider. Jane will email your intake forms after you book; please complete them 24 hours before your visit.

Where do I park?

We’re on the commercial side of The Elmwood apartments. The easiest option is the parking lot behind the building: come in through the main entrance, and our office is right inside on your left.
