Who We Are

This website is operated by Excelsior & Grand Chiropractic, LLC, a Minnesota limited liability company located at 5605 W. 36th St., Suite 100C, St. Louis Park, MN 55416. Care is provided by Dr. Heather Flatgard O’Sullivan, DC, MS, CCSP®, a chiropractor licensed by the Minnesota Board of Chiropractic Examiners.

Information, Not Medical Advice

The content on this website is for general information and education only. It is not medical advice, it is not a diagnosis, and it does not create a doctor-patient relationship. A doctor-patient relationship with Excelsior & Grand Chiropractic begins only when you are examined in our clinic and accepted as a patient.

Never ignore professional medical advice or delay seeking it because of something you read here. If you think you may be having a medical emergency, call 911 or go to the nearest emergency department.

Online Scheduling

Our online scheduling tool is provided by a third-party vendor and is offered for your convenience. An online booking is a request for an appointment; it is confirmed when you receive a confirmation from our office. We may need to adjust the visit type or time based on your needs, and we will contact you if so. Scheduling online does not guarantee that we can accept you as a patient or that a particular treatment is appropriate for you; that is decided at your visit.

Please do not enter detailed health information into the scheduling tool beyond what it asks for. Information you submit through the tool is handled by the vendor under its own privacy terms, and by us under our Notice of Privacy Practices.

Appointments, Cancellations and No-Shows

We hold your appointment time for you. If you need to cancel or reschedule, please give us as much notice as you can, ideally 24 hours, by calling or texting (952) 920-9247. Our current cancellation and no-show policy is provided in your new patient paperwork and is available from our front desk on request.

Text Messages, Email and Phone Calls

When you provide your mobile number, you agree that Excelsior & Grand Chiropractic and the vendors acting on our behalf may send you text messages about your care, including appointment reminders, confirmations, scheduling changes, follow-up after visits and requests for feedback. Message frequency varies. Message and data rates may apply. You can opt out at any time by replying STOP to any message, or by telling our office; reply HELP for help. Opting out of texts does not affect your care, and we will reach you by phone or email instead.

We will not sell or share your mobile number or text-messaging consent with third parties for their own marketing purposes.

Email and text are not fully secure ways to communicate. Please don’t send sensitive health information by email or text; call us or bring it to your visit.

Privacy

How we handle information collected through this website is described in our Website Privacy Policy. How we handle your protected health information as a patient is described in our Notice of Privacy Practices. Both are part of these terms.

Reviews and Testimonials

Patient reviews shown on this site are collected through Google and displayed through a third-party widget. They reflect the experiences and opinions of the individuals who wrote them, are not edited by us, and are not a guarantee of any particular result. Every body is different, and outcomes vary.

Insurance, Pricing and Estimates

Statements on this site about insurance, Minnesota no-fault auto coverage, VA Community Care and pricing are general and may change. Your coverage is verified in our office before care begins, and you are responsible for charges not covered by your plan. If you are uninsured or self-pay, our Good Faith Estimate Notice explains your right to a written estimate.

Links to Other Sites

This site links to third-party websites and services (for example, our scheduling vendor, Google Maps and Google reviews, and referral partners). We don’t control those sites and aren’t responsible for their content, accuracy or privacy practices. A link is not an endorsement.

Unless noted otherwise, the text, photographs, logos and design of this site are owned by Excelsior & Grand Chiropractic, LLC and protected by copyright and trademark law. You may view, print and share pages for personal, non-commercial use with attribution. Any other use requires our written permission. Acceptable Use

Please use this site only for lawful purposes. Don’t attempt to interfere with the site or its scheduling tools, submit false information, scrape content, or use the site to harass or impersonate anyone. We may restrict access for misuse.

This website is provided “as is.” We work to keep it accurate and available, but we don’t guarantee that it will be error-free, uninterrupted or free of harmful components. To the fullest extent permitted by law, Excelsior & Grand Chiropractic, LLC and its owner and staff are not liable for any damages arising from your use of, or inability to use, this website or the third-party services linked from it. These terms are governed by the laws of the State of Minnesota, without regard to conflict-of-law rules. Any dispute about this website will be handled in the state or federal courts located in Hennepin County, Minnesota.
